Enable CUDA v7.0+ use with a Slurm configuration of TaskPlugin=task/cgroup
ConstrainDevices=yes (in cgroup.conf). With that configuration
CUDA_VISIBLE_DEVICES will start at 0 rather than the device number.

Improve logging, comments, and formatting

This change prevents a new debug() message from being generated
if an explicit timeout was set, as used by END_DEBUG3.

Log long processing times with the verbose() call only if they
  take over 3 seconds (previously 1 second).
Log anything over 1 second using debug().
